A radiologist in the top 1% of billers for 88 individual MBS items has agreed to repay $3.6 million and will be reprimanded by the director of the Professional Services Review after their billing practices came the microscope.
They were also disqualified from providing several Medicare item numbers, including 104 for initial specialist attendance and 105 for subsequent attendance, for 12 months.
The specialist was the highest ranked provider of seven MBS items during the review period, including 18225 (continuous infusion or injection of a therapeutic substance to maintain regional anaesthesia or analgesia) and 57963 (orthopantomography for diagnosis or management).
The PSR director reviewed their service patterns, raising concerns “the general body of radiologists” may have considered the practitioner’s “systemic billing and reporting arrangements” was inappropriate.
“The practitioner rendered an unusually high volume of services, including personal attendances at numerous geographically dispersed locations, suggesting it would not always be possible for the relevant MBS requirements to be met or for sufficient clinical input to be provided for each service,” the PSR director’s March update explained.
The radiologist’s clinical input into services was often unclear, while records for initial and subsequent attendance items often did not contain valid referrals for attendance.
“The practitioner acknowledged having engaged in inappropriate practice in connection with rendering MBS Items 104, 105, 18225, 55036, 55065, 55848, 55850, 57341, 57360 and 57963,” the director’s report said.
They agreed to repay $3.6 million and be sanctioned and counselled by the director, and will be barred from providing MBS item 104, 105, 91822, 91823 and 91833 services for 12 months.
Meanwhile, 19 other medical practitioners entered PSR agreements in March 2026, including:
- A GP who agreed to repay $700,000 over concerns about inadequate record keeping, generic templates for GP management plans and team care arrangements, and clinical indications for Amoxicillin + Clavulanic Acid.
- Another who agreed to repay $400,000 to the commonwealth after acknowledging they engaged in inappropriate practice around 8 MBS item numbers, including 585 for urgent after hours attendance.
- A GP who agreed to repay $350,000 after the associate director raised concerns about their record keeping, meeting of MBS requirements and co-billing of Medicare services where not clinically relevant.
